Patient-Generated Health Data

Patient-generated health data (PGHD) is health information you can track between healthcare visits and share with your VA care team.

Take Control of Your Health Between Appointments

Track your health data — like blood glucose, blood pressure, heart rate, steps and pulse oximetry — and share it with your VA care team to make more informed healthcare decisions, spot trends earlier, and manage your health between visits.

Securely Share PGHD With Your VA Care Team

You have the option to securely share your PGHD electronically at any time. Get started in four simple steps:

  1. Work with your care team to decide which health information you track at home—such as symptoms, blood pressure, or activity—is most important, and establish a regular cadence together to review this data and adjust your care as needed.
    • Your VA care team will not be notified automatically when you share data. Develop a plan to review your PGHD with your provider at your next appointment, or send a message through My HealtheVet on VA.gov if you would like your team to review your health data before then.
  2. Track your health data.
    • Use a device, app, or notebook to record information like blood pressure, weight, and blood sugar.
  3. Store your data.
    • Save your data in a secure VA tool, or connect a compatible device that can store data.
  4. Share your data with your VA care team.
    • Send your data before the appointment or when your care team asks for data.

Connect Devices and Apps

VA’s Share My Health Data (SMHD) app enables you to share patient-generated health data  from fitness trackers and watches (such as Google Health, Garmin, and Apple Watch); Bluetooth devices (such as blood glucose, blood pressure monitors, or weight monitors); and health-tracking apps  (such as Apple Health and Android’s Health Connect). You can also manually enter health measurements  (such as blood pressure, oxygen saturation, heart rate, etc.), and then use SMHD to view your health trends, including highs, lows, and averages, over time.

The My VA Images app enables you to securely send photos and videos to your VA care team so you can receive expert healthcare without visiting a VA facility.
When your VA care team authorizes your use of the app as part of your care plan, you will receive an email request to submit photos or videos with a link to My VA Images. The app will guide you through taking the required photos and videos.
NOTE: My VA Images use is initiated by your VA care team. Contact your care team to determine whether My VA Images is the right tool to complement your in-person care.
