By completing a health questionnaire, you help your provider stay informed about your current health and needs — even between visits.
You may be asked to complete a health questionnaire for:
- A variety of topics, including depression, food insecurity, housing status, and posttraumatic stress disorder (PTSD), in advance of an upcoming appointment
- Follow-up care requested by your provider
- Eligibility to participate in national VA health initiatives
When you receive a health questionnaire, you will get a personalized, secure link that takes you directly to the questionnaire without needing to log in. It should be quick to complete, and your answers are confidential and go straight into your VA health record.
How To Know Your Health Questionnaire Message Is Legitimate
